| Below, please find the forms needed
to register for the NJWEA Spring Fling 2012. The Registration Form needs to be filled out by all teams, event coordinators, judges and support staff. Some information may not pertain to you, but I must receive these forms to assure an accurate count for food, T-shirts, etc. The Release form needs to be filled out by every team member who will be participating in the challenge. The Badge Form needs to include all attendees. This includes any family and/or friends. There is a $50.00 non-refundable Team Registration fee this year.
